This report is for new equipment, New Swedish Ladder at outpatient site Lehigh Valley Hospital - Hazleton Rehabilitation at Health Center Mt. Top, located at 237 S. Mountain BLVD, Suite 7, Mt. Top, PA, beginning on February 8, 2024. Lehigh Valley Hospital - Hazleton attested they were in full compliance with the requirements of the Pennsylvania Department of Health's Rules and Regulations for Hospitals, 28 PA Code, Part IV, Subparts A and B, November 1987, as amended June 1998.
